The Lake Cash Club Contest with Punta Cana
OFFICIAL RULES
NO PURCHASE NECESSARY TO ENTER OR WIN. A PURCHASE WILL NOT INCREASE YOUR CHANCE OF WINNING.
1. PROMOTION DATES: The Lake Cash Club Contest with Punta Cana (“Promotion”) will begin at 8:00am CT on January 9, 2012 and end on February 3rd, 2012 or until we make four (4) Punta Cana Winners whichever comes first.
2. ELIGIBILITY RESTRICTIONS
a. The Promotion is open to all legal residents of the United States who are 18 years of age or older at the time of entry and who live in Wisconsin in one of the following counties: Washington, Waukesha, Ozaukee, Racine, Milwaukee, Kenosha, Jefferson, Sheboygan, Dodge, Fond du Lac, Manitowoc or Walworth County in Southeastern Wisconsin. Employees of Journal Broadcast Group (“Journal” or “Promoter”), d/b/a WLWK-FM, Hard Rock-Punta Cana, Funjet Vacations their corporate parent, subsidiary, and affiliated entities, their advertising agencies, participating sponsors/promotional partners, other broadcasting stations in the Milwaukee Market, and the members of the immediate families (spouse, parents, siblings or children) or households of each of them are ineligible to participate or win. This Promotion is subject to all applicable federal, state, and local laws and regulations. Void outside the United States and where prohibited.
b. Only one winner per household is permitted in the Promotion.
c. Each contest winner will be required to fill out Journal Broadcast Group’s prize claim form and present a valid form of government-issued photo ID.
d. Entrants are required to provide truthful information and Promoter will reject and delete any entry that it discovers to be false or fraudulent. Promoter will disqualify any entry from individuals who do not meet the eligibility requirements, and Promoter will also delete any entry knowingly received from persons under the age of 13 in compliance with the Children’s Online Privacy Protection Act.
3. ENTRY
a. To participate in the Promotion, you must be a member of the Lake Cash Club and listen for your name to be announced on 94.5 Lake FM at 8AM, 1PM or 5PM each weekday. If it is announced you must call the station at 414.799.9494 within 20 minutes. If the caller can correctly identify his/herself they win $100 and their name will be entered to win a weekly drawing for a trip for (2) to Punta Cana, Dominican Republic.
b. All contestants who call back to claim the $100 prize will be entered into the Punta Cana Trip drawing and (4) winners will be randomly drawn from the $100 contestants who called back within 20 minutes on or about, January 6th, 13th, 20th or 27th.
c. Anytime you call the WLWK-FM studios you must assume that your call will be broadcast or recorded and you grant us permission to broadcast the call or record it for later broadcast on 94.5 WLWK-FM and on the station’s internet stream at www.945lakefm.com.
d. Due to delays in the station’s online streaming of its broadcast signal, listeners to the online stream may not be able to participate in or may be disadvantaged in participating in on-air contests.
4. PRIZES:
a. Daily Prize: A maximum of 60 $100 prizes in the form of a check from Journal Broadcast Group will be awarded to winners of this Promotion, unless the Promotion does not generate 60 winners by the time its end date occurs on January 27, 2012.
b. Weekly Prize: One (1) trip for two (2) to the Hard Rock Hotel & Casino Punta Cana, Dominican Republic, Minimum 4 night stay, All inclusive food and drink to be awarded in our grand prize drawing each week from January 2- 27, 2012.
c. A total of 4 grand prizes are available. If 4 grand prizes are awarded prior to January 27, 2012 the Hard Rock Punta Cana Promotion will end immediately following the award of the fourth grand prize, with no further on-air opportunities to win.
d. No other prizes will be awarded and no prizes may be substituted or transferred except that Promoter may, at its sole discretion, substitute prizes of comparable value or cash. The prizes are expressly limited to the item(s) listed above and do not include taxes or any other expenses, including, but not limited to, advertising production expenses. Other restrictions may apply.
5. WINNER SELECTION AND NOTIFICATION
a. Decisions of Promoter management with respect to the Promotion are final.
b. If the Lake Cash Club winner calls the station within 20 minutes at 414.799.9494 after their name is announced and is confirmed by the Promoter host they win $100 and are entered to win the Grand Prize trip to Punta Cana.
c. Each winner will be contacted and notified by phone or email regarding the method for claim their prize. If a potential winner cannot be contacted, fails to complete, sign and return any affidavit of eligibility or liability and publicity release, or if a prize or prize notification is undeliverable, that potential winner will be disqualified and forfeit any claim for any prize.
d. Odds of winning $100 depend upon the number of people entered in the Lake Cash Club and your name being called at 8am, 1pm, or 5pm, and listeners attempting to call following each cue to call. Odds of winning the Punta Cana trip for (2) depend upon the number of failed $100 prize contestants that are eligible for the Punta Cana trip.
e. Unless otherwise specified in the prize notification, all prizes or prize certificates may be picked up at the office of the Promoter at the address below. 720 East Capitol Drive Milwaukee, WI 53212. The winner will forfeit any prize or prize certificate not claimed within thirty (30) days of winning. Prize or prize certificates will not be mailed to the winner. The Promoter, its sponsor, or promotional partners are not responsible for the safe arrival of a prize or prize certificate.
f. Prize checks will take approximately four to six weeks to be processed before they will be ready to be claimed.
6. CONDITIONS
a. Payments of all federal, state and local taxes are solely the responsibility of the winners. Winners will be required to complete and submit an IRS Form W-9 or the equivalent including the winner’s full Social Security Number for receipt of any prize valued at $600 or more or for any prizes awarded by Promoter in a calendar year with an aggregate value of $600 or more. Failure to submit a complete W-9 or equivalent will result in disqualification and forfeiture of the prize. Winners of trips will fill out Travel Release forms in adjacent to 1099 Tax Release forms.
b. By participating in the Promotion, each winner agrees, and by accepting a prize, agrees, to have his, her, or its name, city, state, voice, or likeness used in any advertising or broadcasting material relating to this Promotion without additional financial or other compensation, and, where legal, to sign a publicity release confirming such consent prior to acceptance of the prize.
c. Prior to awarding any prize or prize certificate, and depending on the nature of the prize and eligibility requirements of the contest, the Promoter in its sole discretion will require verification of Promotion winner’s identification or the identification of any individual executing any document on behalf of the winner by a showing of valid government-issued photo identification. By participating in the Promotion, each winner agrees, and by accepting a prize, to release Promoter, its sponsor(s) and promotional partner(s), and each of their parent, subsidiary and affiliated entities, and the officers, shareholders, directors, employees, agents and representatives of each of them (collectively, the “Released Parties”) harmless against any and all claims or liability arising directly or indirectly from the prize or participation in the Promotion.
d. Promoter, in its sole discretion, reserves the right to disqualify any person (and all of their entries) from this Promotion if he or she tampers with the entry process. Promoter further reserves the right, at its sole discretion, to modify, cancel, terminate or suspend the Promotion, or any part of it, if it is not capable of completion as planned or if any fraud, technical failures or any factor beyond the Promoter’s control, including infection by computer virus, bugs, tampering, unauthorized intervention or technical failures of any sort, corrupt or affect the administration, security, fairness, integrity or proper conduct of the Promotion as determined by Promoter in is sole discretion. Any attempt by an entrant or any person to deliberately damage any Promoter web channel or to undermine the legitimate operation of this Promotion may be a violation of criminal and civil laws. Should such an attempt be made, Promoter reserves the right to seek full prosecution and/or damages from any such individual to the fullest extent permitted by law. Promoter’s failure to enforce any term of these Official Rules shall not constitute a waiver of this provision.
e. The Released Parties are not responsible for (i) typographical or other errors in the printing, the offering or the administration of the Promotion, or in the announcement of a prize; (ii) entries not received due to difficulty accessing the internet, service outage or delays, computer difficulties, malfunctions, disconnections, other technological failures, telephone service outages, delays, dropped calls, or busy signals; or (iii) lost, stolen, mangled, misdelivered, postage due, illegible, incomplete, incorrect, or late entries.
f. To obtain a copy of the Official Rules or a list of winners following completion of the Promotion (please specify which), send a self-addressed, stamped envelope (VT residents may omit return postage) specifying “Official Rules” or “Winner List” to THE LAKE CASH CLUB CONTEST with PUNTA CANA or Official Rules Request, WLWK 720 E Capitol Dr. Milwaukee, WI 53212. A copy of the Official Rules and a list of winners (when complete) are also available during regular business hours at WLWK 720 E Capitol Dr. Milwaukee, WI 53212. All such requests must be received within thirty (30) days following completion of the Promotion.
g. As a licensee of the FCC and trustee of the public airwaves, 94.5 WLWK reserves the right to interrupt and/or discontinue any contest or promotion (and not award the offered prize) at the discretion of 94.5 WLWK management if world events, the national mood, or public safety so warrant.
h. By participating in this contest, entrants agree to be bound by the Official Rules and the decisions of the judges.
i. 94.5 WLWK reserves the right to amend the rules to any contest at any time.
j. The decision of 94.5 WLWK management is final.
7. TERMS OF USE AGREEMENT & PRIVACY POLICY
